Background

5-methylcytosine (5-mC) is one of the most important epigenetic modifications in DNA, formed primarily by the addition of a methyl group to the fifth carbon atom of cytosine, and in mammalian genomes it predominantly occurs at CpG dinucleotide sequences. Catalyzed by DNA methyltransferases (DNMTs), this modification plays critical roles in gene regulation, embryonic development, genomic imprinting, and X-chromosome inactivation, and is generally associated with transcriptional repression—methylation of CpG islands in promoter regions impedes transcription factor binding or recruits methyl-binding proteins and histone-modifying enzymes, thereby establishing a compact chromatin structure that silences gene expression; however, its function is context-dependent, as intragenic 5-mC can actually promote transcriptional elongation or alternative splicing. Aberrant 5-mC patterns—such as global hypomethylation or promoter-specific hypermethylation—are closely linked to cancer, neurodegenerative diseases, and developmental disorders, and it also serves as the core substrate for active demethylation, being oxidized by the TET protein family into intermediates like 5-hydroxymethylcytosine (5-hmC) that ultimately lead to DNA demethylation, thus dynamically regulating gene expression networks. As the "fifth base" in life sciences, 5-mC not only carries an additional layer of information beyond the genetic sequence, but also acts as a molecular bridge connecting internal and external signals in cellular differentiation, disease pathogenesis, and environmental responses, making it a cornerstone of epigenetic research.
