Background
Alkaline phosphatase (ALP) is a ubiquitous membrane-bound metalloenzyme that catalyzes the non-specific hydrolysis of phosphate monoesters at alkaline pH, existing as four distinct human isozymes—tissue-non-specific (liver/bone/kidney), intestinal, placental and germ-cell—whose differential glycosylation and post-translational modifications generate characteristic electrophoretic patterns exploited clinically to pinpoint the source of elevated serum activity that serves as a sensitive but non-specific biomarker for hepatobiliary obstruction, bone formation by osteoblasts, pregnancy, malignancy, or metabolic disorders, while intracellularly the enzyme participates in lipid transport, dephosphorylation of receptors, and modulation of inflammatory signaling through its ectophosphatase activity on extracellular ATP.
