Background
ALDH5A1 encodes a mitochondrial NAD⁺-dependent succinate semialdehyde dehydrogenase (SSADH), a key enzyme in the catabolic pathway of the neurotransmitter GABA, responsible for the irreversible oxidation of succinate semialdehyde to succinate. Deficiency of this gene leads to a rare autosomal recessive disorder, succinate semialdehyde dehydrogenase deficiency (also known as 4-hydroxybutyric aciduria), in which GABA metabolism is blocked and the neurotoxic metabolite γ-hydroxybutyric acid (GHB) accumulates abnormally, resulting in widespread neurological damage including developmental delay, ataxia, seizures, behavioral disorders, and intellectual disability. Recent studies have revealed that the functional significance of ALDH5A1 extends far beyond its classical metabolic role. In glioblastoma, this gene is significantly downregulated under hypoxic conditions via negative regulation by miR-210, and this downregulation promotes tumor proliferation by reprogramming cellular metabolism and maintaining a high glycolytic phenotype, with low expression closely associated with poor patient prognosis. Conversely, ALDH5A1 overexpression inhibits reactive oxygen species production and shifts cellular metabolism from glycolysis to oxidative phosphorylation, thereby suppressing tumor growth. Additionally, in a mouse model of SSADH deficiency, loss of this enzyme not only impairs GABA metabolism but also leads to astrocyte dysfunction and severe depletion of myelinSphingolipid, suggesting that glial pathology is an important component of the disease mechanism. These findings reposition ALDH5A1 from a conventional metabolic enzyme to a multifunctional regulatory hub linking neurotransmitter metabolism, glial cell function, and tumor metabolic reprogramming.
