CCL17 as a Cardiac Remodelling Target: Reading the Evidence Ladder

CCL17 from dendritic cells acting through CCR4 on Th2 cells to drive cardiac hypertrophy, and the effect of blocking the axis

Pathological cardiac remodelling, defined by left ventricular hypertrophy, myocardial fibrosis and inflammation, decides how heart failure progresses. Anti-inflammatory strategies have improved outcomes in some settings, but the specific mediators worth blocking remain contested. CCL17, also called TARC, has moved from a peripheral observation to a candidate driver of that process. What makes the case interesting is not any single experiment. It is the sequence, with each layer closing a gap the previous one left open.

From Atherosclerosis to the Failing Heart

Chemokines and their receptors coordinate immune cell migration and maintain homeostasis. CCL17 signals mainly through C-C chemokine receptor 4, and it is chemotactic for T lymphocytes. It was first characterised as an important regulator of atherosclerosis.

That history shaped the hypothesis. If CCL17 participates in vascular injury, it may participate in every stage of the cardiac response to injury. The proposition was testable, and the work that followed tested it at several levels of organisation.

Layer One: Association in Human Tissue and Blood

Transcriptomic comparison of 366 heart failure and non-failure samples showed CCL17 expression significantly increased in failing myocardium. Serum CCL17 was also significantly higher in heart failure patients than in controls. Tissue and circulation agreed, which is the minimum requirement for a circulating biomarker claim.

Association alone proves little, however. A marker can rise because it drives disease, or because disease produces it. The next layers were designed to separate those possibilities.

Layer Two: Age, a Confounder That Became a Finding

Aptamer-based profiling of 2,925 plasma proteins in 4,263 individuals aged 18 to 95 showed CCL17 strongly associated with age. Inflammatory and cytokine pathways were the most enriched among the associated proteins. A separate analysis of 811 subjects reproduced the age relationship for CCL17 specifically.

Age is usually treated as a nuisance variable to be adjusted away. Here it became part of the biology. Supporting evidence came from mice, where serum CCL17 rose with age while CCL22, another CCR4 ligand, did not. Two ligands for the same receptor diverging across lifespan implies independent regulation, and independent regulation implies a specific function rather than a general ageing effect.

Layer Three: Dynamic Change Tracks Cardiac Function

Seventeen patients with acutely decompensated heart failure were sampled before and after treatment. CCL17 fell as cardiac function recovered. A marker that moves with clinical improvement within an individual is stronger evidence than a difference between groups, because each patient serves as their own control.

In mice, angiotensin II infusion increased CCL17 at both mRNA and protein level. The pharmacological model therefore reproduced the human direction of change, which allowed mechanistic work to proceed in animals with some confidence.

Layer Four: Genetic Deletion

Ccl17 knockout mice were generated with CRISPR/Cas9. Deletion was confirmed functionally, with no detectable CCL17 in serum or cardiac tissue. Aged knockout animals appeared younger than wild-type controls and performed better on motor function and spatial working memory tests. Loss of a chemokine improving general ageing phenotypes was an unexpected result and broadened the interpretation.

Structure and Function

Echocardiography in aged mice showed that Ccl17 deletion improved systolic function. Heart weight normalised to tibial length and to body weight was lower in aged knockout animals than in wild-type. Haematoxylin and eosin staining, wheat germ agglutinin staining and Sirius red staining showed reduced cardiac enlargement and reduced fibrosis in the knockout group.

In the angiotensin II model, four weeks of infusion raised blood pressure in both genotypes while heart rate stayed unchanged. Ejection fraction and fractional shortening improved in knockout animals, and heart weight ratios fell. Cardiomyocyte cross-sectional area and interstitial fibrosis were both reduced.

The Gene Programme

Quantitative PCR showed suppression of cardiac function genes, hypertrophy genes and fibrosis genes in knockout animals. The structural phenotype was therefore accompanied by a transcriptional phenotype, which is what distinguishes a real remodelling effect from a haemodynamic artefact.

Layer Five: Pharmacological Blockade

Genetic deletion establishes necessity. It does not establish that blocking the molecule in an adult animal will help. That required an antibody. Mice received angiotensin II together with an anti-CCL17 neutralizing antibody or an isotype control.

Serum CCL17, elevated by angiotensin II, fell with the neutralizing antibody. Compared with isotype-treated animals, blocked mice showed improved cardiac function, smaller cardiomyocyte area and less fibrosis. Flow cytometry showed reduced CD45 positive and CD3 positive CD4 positive infiltration, fewer Th2 and Th17 cells, and more Th1 and Treg cells.

Mechanism: Receptor, Cell and Balance

The pathway now reads as follows. Dendritic cells secrete CCL17. CCL17 engages CCR4 on T helper cells and recruits Th2 cells into the myocardium. That recruitment distorts the T cell balance, lowering the Th1 to Th2 ratio and the Th17 to Treg ratio, and promotes release of profibrotic factors. Hypertrophy, fibrosis and eventually failure follow.

The T cell data support each step. In angiotensin II treated hearts, CD4 positive T cells, macrophages, dendritic cells and neutrophils all increased, and all were reduced in knockout animals. Protein array profiling showed Th2 cytokines down and Th1 cytokines slightly up in knockout mice. The shift is coherent across cell counts, cytokine output and structure.

What This Means for Measurement

A moving target requires a measurement that can follow it. Serum CCL17 is the accessible compartment, and the studies above used it at every stage: cohort comparison, age association, treatment response, and blockade verification.

Species matching is the practical constraint. The paired antibody kit validated for this work is configured for monkey serum, plasma and cell culture supernatant. That makes it suitable for non-human primate studies and for translational work in which a mouse model cannot answer the question. One caution belongs in every protocol. CCL17 and CCL22 share CCR4 but are regulated differently, so an assay must not report one as the other.

The one-step format shortens the interval from loading to reading, which matters when a study runs large sample series or when results inform a same-day decision. The high-sensitivity configuration extends the lower end of the curve, where baseline CCL17 in healthy animals tends to sit.

Reading the Evidence Carefully

The case for CCL17 is strong but not closed. Human data are associative, the interventional evidence is murine, and no trial has yet tested CCL17 blockade in patients. What the work does establish is a coherent mechanism with a measurable circulating readout and a plausible therapeutic entry point.

For laboratories entering this area, three decisions shape the result. Sample type should be fixed early, because serum and plasma are not interchangeable across serial timepoints. The assay platform should stay constant within a study, since values do not transfer between formats. And CCL22 should be measured alongside when the question concerns CCR4 biology rather than CCL17 alone. Reagents described here are supplied for research use only.
