Background

CD124, also known as interleukin-4 receptor alpha (IL-4Rα), is a ~140 kDa type I transmembrane glycoprotein of the class I cytokine receptor family that serves as the ligand-binding subunit for both interleukin-4 and interleukin-13: upon IL-4 ligation it pairs with the common γ-chain (CD132) to form the type I IL-4 receptor on hematopoietic cells or with IL-13Rα1 to form the type II receptor on non-hematopoietic cells, thereby activating JAK1/3–STAT6 signaling that drives Th2-cell differentiation, B-cell IgE class switching, alternative macrophage activation, mucus production and allergic inflammation, while a soluble ectodomain generated by alternative splicing or metalloprotease shedding retains ligand-binding capacity and functions as a decoy receptor that inhibits IL-4-mediated T-cell proliferation and IL-5 up-regulation; genetic polymorphisms and dysregulated expression of CD124 have been linked to atopy, asthma, HIV-1 resistance and tumor progression, making it a prominent therapeutic target in type 2-biased inflammatory diseases.
