Background
P4HB, also known as protein disulfide isomerase (PDI) or PDIA1, is a multifunctional endoplasmic reticulum (ER) protein with a molecular weight of approximately 57 kDa. It contains four thioredoxin-like domains (a, b, b′, and a′), with the a and a′ domains harboring the catalytic CGHC motif. Adopting a horseshoe-shaped conformation and bearing a KDEL ER-retention signal at its C-terminus, it is one of the most abundant marker proteins in the ER. As the β subunit of the prolyl 4-hydroxylase complex, it forms an α2β2 tetramer with α subunits and participates in the 4-hydroxylation of proline residues in procollagen, which is essential for stabilizing the collagen triple helix. Meanwhile, functioning as a disulfide isomerase/oxidoreductase, it catalyzes the formation, breakage, and rearrangement of disulfide bonds in nascent secretory and membrane proteins, thereby maintaining ER protein folding and quality control. P4HB also exhibits concentration-dependent chaperone activity, inhibiting the aggregation of misfolded proteins at high concentrations or after phosphorylation by FAM20C. Additionally, it can bind thyroid hormones, participate in nitric oxide transport via S-nitrosothiols, and serve as a structural subunit of the microsomal triglyceride transfer protein complex. Beyond its ER functions, a fraction of P4HB is present on the cell surface or extracellularly, where it regulates the redox state of surface proteins such as integrins, contributing to platelet activation, fibrin formation, and thrombosis, and is implicated in pathological processes including tumor progression, neurodegenerative diseases, and Cole-Carpenter syndrome.
