FBLN7 in Post-Infarction Remodeling: From a Circulating Signal to the EGFR–FAK/AKT Axis

FBLN7 in Post-Infarction Remodeling: From a Circulating Signal to the EGFR–FAK/AKT Axis

Fibulin 7 entered the cardiac literature from an unusual direction. It was characterized in renal tubular calcification, tumor metastasis and angiogenesis before anyone asked whether it participates in myocardial fibrosis. The answer, assembled from transcriptome data, genetic loss and gain of function, and a receptor-level mechanism, is that it does.

Why Post-Infarction Fibrosis Decides the Outcome

An infarct heals in two phases that are easy to confuse. Early repair replaces necrotic myocardium with a collagen-rich scar, and that replacement is protective. Persistent fibroblast activation is the second phase, and it is not protective.

Collagen continues to deposit in non-infarcted regions, the ventricle dilates, systolic function falls and the risk of heart failure rises. The cellular event at the center of this transition is the conversion of cardiac fibroblasts into myofibroblasts. Myofibroblasts express alpha-SMA and vimentin and drive synthesis of collagen I and collagen III. Molecules that control that conversion are therefore candidate targets.

Fibulin 7 as a Secreted Matrix Protein

The fibulin family consists of secreted proteins that participate in extracellular matrix assembly and in cell-matrix interaction. Several members are already implicated in cardiovascular pathology: ischemia-reperfusion injury, cardiac hypertrophy and vascular remodeling.

FBLN7 is a newer member with strong adhesion-protein character. Its documented roles include renal tubular calcification, tumor metastasis and angiogenesis. Whether it touched myocardial fibrosis was an open question until the datasets below were assembled.

Three Independent Lines of Evidence That FBLN7 Rises

First, transcriptomes. Public heart tissue datasets from hypertrophic cardiomyopathy and dilated cardiomyopathy showed FBLN7 mRNA significantly upregulated against normal controls. GO and KEGG analysis placed the differential genes in cell-extracellular matrix interaction, which is the expected neighborhood for a fibrotic process.

Second, human serum. FBLN7 is secreted, so it does not stay local. Serum FBLN7 in patients with myocardial infarction was significantly higher than in controls.

Third, animal models. In mouse myocardial infarction, serum FBLN7, cardiac FBLN7 mRNA and cardiac FBLN7 protein were all elevated. An isoproterenol-induced fibrosis model showed the same pattern. Three independent readouts converging on one direction is stronger evidence than any single one.

Loss of Function Reduces Dilation and Fibrosis

Fbln7-deficient mice were compared with littermate controls after infarction or sham surgery. Knockout animals showed reduced left ventricular dilation, and the decline in systolic function was smaller. Myocardial fibrosis and excessive collagen deposition were significantly reduced.

The result matters because it separates correlation from causation. A molecule that merely rises with fibrosis would not produce this phenotype when removed. It also narrows the interpretation of the serum data. Elevated circulating FBLN7 is not only a consequence of matrix turnover but can be part of the driving signal.

Gain of Function Worsens Remodeling

The reciprocal experiment used adeno-associated virus AAV9 to overexpress FBLN7 in the mouse heart. Infarction-induced dilation, systolic decline, fibrosis and collagen deposition all worsened, which mirrors the knockout phenotype in the opposite direction.

At the cellular level, FBLN7 deficiency reduced the post-infarction upregulation of alpha-SMA and vimentin, while overexpression enhanced it. Confocal imaging showed myofibroblasts and FBLN7 co-localizing in the infarct region. FBLN7-positive transport vesicles also appeared in cardiac fibroblasts isolated from infarcted hearts.

Together these observations place FBLN7 inside the fibroblast, not merely around it. A secreted protein that is taken up and trafficked in vesicles can act on the cell that produced it. That is a different pharmacology from a purely structural matrix component.

The FBLN7–EGFR–FAK/AKT Axis

Focal adhesion kinase coordinates adhesion, migration and matrix remodeling. Pathway enrichment and prior work both pointed at FAK/AKT in hypertrophy and fibrosis. Loss or knockdown of FBLN7 inhibited TGF-beta1-stimulated phosphorylation of FAK and AKT, and overexpression enhanced pathway activation.

Co-immunoprecipitation then failed to detect direct binding between FBLN7 and FAK. That negative result is informative, because it places FAK downstream instead of at the receptor.

The receptor turned out to be EGFR. FBLN7 binds EGFR and regulates its tyrosine kinase activity, and EGFR activation initiates the FAK and AKT cascades. Gefitinib, an EGFR tyrosine kinase inhibitor, reduced phosphorylation at EGFR Y1068 and Y1173. It also weakened the ability of FBLN7 overexpression to activate FAK/AKT and to drive fibrotic responses.

Domain mapping placed the interaction in the EGF-like and EGF-CA domains of FBLN7. That assignment matters for reagent design, because an antibody raised against the wrong domain will not interfere with the interaction and will not work as a blocking tool.

Choosing the Right Matrix for FBLN7 Measurement

FBLN7 is both a matrix-associated protein and a secreted one, so the matrix you sample determines what you learn. Serum and plasma report the circulating pool and suit cohort comparison and time-course studies. Cell culture supernatant reports secretion from fibroblasts under stimulation. Tissue lysate reports local abundance and is the right matrix for mechanism work.

The three do not move in lockstep. A treatment that blocks secretion without changing transcription will look negative in tissue and positive in supernatant. Collect matched samples where the design allows, and record the interval between collection and processing for each one.

Pair the ELISA with alpha-SMA, vimentin, collagen I, collagen III, phospho-FAK, phospho-AKT and phospho-EGFR. Reading one node tells you a level. Reading the axis tells you whether the pathway is engaged.
